DentalSlip Terms of Use
Version 2026-10-15

These terms are the product agreement for a DentalSlip subscription between the subscribing office and UCPlaces LLC, a Virginia limited liability company with its principal office at 303 Maple Ave W, Suite F, Vienna, VA 22180, which operates DentalSlip. Have them reviewed by counsel before production use.

1. The service. DentalSlip lets a dental office design referral forms, publish them, and receive submissions in an inbox. It is not a practice management system, insurance processor, or electronic health record.

2. The customer. The subscribing office is the customer. Referring offices and patients do not buy a subscription and do not receive an account in this release.

3. Plans. Basic is 19 US dollars per office per month and includes up to 10 live forms. Pro is 30 US dollars per office per month and includes up to 20 live forms. Both plans include up to 20 saved templates and unlimited referral submissions. Taxes, if any, are additional. There is no annual plan in this release.

4. Acceptable use. Do not share logins. Do not attempt to access another office’s records. Do not upload malware. Do not use the service for anything other than dental referrals the office is authorized to receive.

5. Patient information. The office decides what to collect. The office is responsible for its staff, its website, and printed copies. DentalSlip processes submissions to provide the inbox, notifications that omit clinical content, and PDF copies.

6. Availability. The service is provided as available. Maintenance and failures can interrupt collection. A submission is received only after the office sees a receipt number. Printing is not delivery.

7. Cancellation. Cancellation takes effect at the end of the paid period. Existing records remain available to authorized users for export and review according to the BAA. Unpaid status does not delete records.

8. Contact. Use the contact form for account questions. Do not include patient names or clinical details in support messages.

9. Disputes. Any dispute arising out of or relating to these Terms or the BAA will be resolved by binding arbitration administered by the American Arbitration Association under its Commercial Arbitration Rules, before a single arbitrator, seated in Fairfax County, Virginia. Either party may bring an action in the state or federal courts located in Fairfax County, Virginia to compel arbitration, enforce an award, or seek urgent injunctive relief, and each party consents to those courts' jurisdiction and venue. Claims will be brought only individually, not as a class.

10. Governing law. These Terms are governed by the laws of the Commonwealth of Virginia, except to the extent federal law applies.

11. Limitation of liability. To the maximum extent permitted by law, each party's total liability arising out of or relating to these Terms or the BAA will not exceed the fees paid by the subscribing office to UCPlaces LLC in the twelve (12) months before the event giving rise to the claim, and neither party will be liable for indirect, incidental, special, consequential, or punitive damages. This limitation does not apply to a party's indemnification obligations, if any, or liability that cannot be limited by law.
