FAQ

Do referring offices need an account?
No. A referring dentist or their staff open the form, fill it out, and submit. They are not asked to create a login.
How much does DentalSlip cost?
Basic is $19 per office per month and includes 10 live forms, 20 saved templates, and 5 users. Pro is $30 per office per month and includes 20 live forms, 40 saved templates, and 10 users. Group is $55 per office per month and includes 100 live forms, 200 saved templates, and 25 users. Additional users are $3 per user per month. Every plan receives unlimited referrals. Referring offices are never charged. Sending a document for signature is unlimited on every plan. Live forms (referral or e-signature) are shared. An unpublished signature template, or a document that is only sent, does not use a live form. Basic documents are up to 40 MB and 100 pages. Pro documents are up to 100 MB and 500 pages. Group documents are up to 250 MB and 1,000 pages.
What happens if I have more forms than my plan allows?
Drafts and archived forms do not count as live. Publishing stops when you reach the live limit. Archive or unpublish a form to free a slot. Existing referrals stay in Incoming Forms. Saved templates have their own limit. Switching to a smaller plan is blocked while you have more live forms or saved templates than that plan includes, or a document with more pages than it allows. Archive or unpublish until you are within those limits, then change the plan. Live forms (referral or e-signature) are shared. Sending a document for signature does not use a live form.
Can patients still leave with a paper copy?
Yes. Print blank form is the designed page with empty lines and no submission banner. Print without submitting is marked Not submitted online. Submit and print includes the receipt. Printing never means the referral was delivered.
What files can be attached?
PDF, JPG, and PNG. Up to 10 files, 20 MB each, and 100 MB per referral. Other formats, including archives and DICOM, are declined. Larger imaging studies should use the practice’s existing transfer method.
Will the notification email contain patient information?
No. Verified recipients get a short note that a new referral is waiting, with a link to sign in. The patient’s name, clinical notes, and files stay in Incoming Forms.
Is DentalSlip HIPAA certified?
No one can claim HIPAA certification from HHS. DentalSlip is built for HIPAA. The Business Associate is UCPlaces LLC, and the product includes a Business Associate Agreement with breach notice within 30 calendar days and a written retention schedule. Compliance also depends on hosting, subprocessors, a risk analysis, and how the practice uses the product. Obtain legal review before collecting real patient information.
How long does a sign-in last?
A session ends after 60 minutes without activity, or after 12 hours, whichever comes first.
Does this replace my practice management software?
No. DentalSlip is one secure place for incoming referrals. It does not schedule appointments, bill patients, or store a full chart. It sits beside the software you already use.
